Release Forms

Photo Release Form (12k PDF*) » This form grants permission for an individual’s photo to be used by the University for marketing purposes in print, electronic, or video format. Please note: Signed photo release forms are not needed when subjects are in public places, such as outdoor campus locations. Photographs or videotaping of minors (those under age 18) in preschool or youth camps must be done only with school or camp permission.

AV Release Form (13k PDF*) » This form grants permission for a speaker to be recorded for use in a Webcast or podcast placed on the University Web site. This form must be completed by any speaker or interviewee recorded for these purposes. 

Both forms should be forwarded to Office of University Communications, lower level, Elliott Hall.
