Release Forms

Photo/Video/Audio Release Form (131k PDF*) » This form grants permission for an individual’s appearance in a photo or recorded video/audio clip to be used by the University for marketing and communication purposes in print, electronic, or video format. Photographs or videotaping of minors (those under age 18) requires parental consent, or prior consent through release forms from the minor’s school or camp. Please note: Signed photo release forms are not needed when subjects are in public places, such as outdoor campus locations.

Webstream/Lecture/Presentation AV Release Form (131k PDF*) » This form grants permission for a speaker to be recorded for use in a Webcast, podcast, or video placed on the University Web site. This form must be completed by any speaker or interviewee recorded for these purposes.

Both forms should be forwarded to Office of Marketing and Communication, lower level, Elliott Hall.
