OWU Publicity Release Form (132k PDF*)

This form grants permission for a person’s appearance in a photo or recorded video to be used by the University for its print, electronic, or video communications. Parental consent is required for minors (those under the age of 18), unless there is prior consent through release from their school or camp. Please note that signed photo release forms are not needed when subjects are in “public” places, such as outdoor campus locations.

This form also provides permission from a scheduled speaker or interviewee for any event that is recorded or webcast by the University.

Please return this form as soon as possible to the Office of Marketing and Communication in Elliot Hall.
